Immunization records from their provider (please provide all that have been received so far, even if they are scheduled to finish the series sometime before school starts). Maine law requires students to be fully vaccinated, or have a medical exemption in place prior to their first day of school. Please upload records to PowerSchool enrollment.
Physical exam (printed notes from their last physical exam). Please upload records to PowerSchool enrollment.
If your student has life threatening allergies, asthma, or any other medical condition, please email jlizanecz@yorkschools.org to schedule a telephone conference ASAP. Telephone conferences are available 3/13-6/15
